Welcome to the Team Diagnostic Survey Site

The Team Diagnostic Survey (TDS) is an instrument taken by members of a work team to diagnose the how well the team is structured, supported, and led. It also provides an assessment of how well members work together, and of members' motivation and satisfaction.

When team members have completed the TDS, a feedback report is automatically generated. This report, which members can view on the web using their Team ID and Password, graphically shows the team's special strengths, areas where improvement may be needed, and how the team compares to other teams that previously have taken the instrument.

  • The TDS is based on the conditions for team effectiveness discussed in the book "Leading Teams: Setting the Stage for Great Performances," by J. Richard Hackman.

Due to technical problems on this server, as of March 6, 2014, we will not be signing up new teams and projects on this website. We will continue to support any teams already in process to complete the survey and receive their reports.

The new Team Diagnostic Survey website will be available by April 15, 2014. Meantime, if you would like to inquire about use of the Team Diagnostic Survey, please contact Trexler Proffitt at:

We apologize for the delay, and thank you for your interest in the Team Diagnostic Survey.
